Have the institution classify the proposed product in writing:

Is it already approved and marketed in China for this intended use?
Is it a specially permitted imported product for a named patient at a designated Lecheng institution?
Is the patient also being invited into a real-world research project or another study?

Real-world research can feed later regulatory evaluation, yet it differs from routine care and proves nothing about benefit for this particular patient. Hainan’s service guide requires a research plan, investigator information, data-quality arrangements, and other formal materials [3]. If research comes up, ask for a separate explanation of purpose, consent, data use, additional visits, withdrawal, injuries, and costs.

Filter three: product-to-patient evidence

Hainan’s official international-access page lists Haikou Meilan, Sanya Phoenix, and Qionghai Boao as distinct airports with different addresses [6]. Pick the gateway only after the hospital address is confirmed. Then check whether the actual flight is international or domestic, the terminal requirements, the ground transfer, and whether the patient can tolerate the journey.

The island railway and road network connect cities on a timetable; a scheduled train is no substitute for a clinical transfer. Before treatment, document:

arrival airport or port and exact terminal;
door-to-door transfer time with luggage and reduced mobility;
whether the patient can sit, walk, or travel after the procedure;
nearest appropriate emergency department;
whether a complication can be treated locally or requires transfer to another city;

allowed walking distance and use of stairs;
wound, drain, device, or infusion care;
temperature, hydration, sun, and infection precautions;
required laboratory or imaging checks;
warning signs and response time;
number of nights near the treatment institution;
fit-to-fly or long-journey criteria;
responsible clinician after leaving Hainan.

Inspect the accommodation itself: step-free entry, lifts, bathroom safety, medicine refrigeration, reliable power and communication, appropriate food, and an emergency route. When daily monitoring or rapid reassessment is needed, a remote beachfront property works against you.

Make the quote auditable

Split the estimate into clinical care, special product, research-related procedures, travel, and recovery. Every payment needs a named legal recipient. Ask whether the product must be paid before import, what happens if permission or eligibility fails, which complications are excluded, and whether insurer pre-authorisation covers the exact institution and product.

The phrase “international advanced medicine” should never stand in for an itemised quote. A special product can add pharmacy, customs, monitoring, replacement, data, and follow-up obligations.

Close the island handoff

Before departure, collect the final diagnosis, treatment record, product identity and batch or serial details, consent, adverse-event instructions, laboratory and imaging results, medicines, restrictions, pending-result owner, and contact hours. If the patient contributed real-world data, also keep the study identifier, consent version, research contact, withdrawal route, and schedule.
