A negative report also has a defined testing range and method; it does not exclude every possible change. Add other prescriptions, over-the-counter products, herbal remedies, and supplements. Anticoagulants, antiplatelet medicines, and recent anti-infective treatments should not disappear from the record because the consultation concerns lymphoma. A medication review needs to know what is actually being used, including products that the patient does not regard as medicine.
Current acalabrutinib labeling illustrates why this matters: interactions can change drug exposure, and bleeding and other adverse effects require clinical attention.[6] The action depends on the particular product, accompanying medicines, and individual circumstances. Compiling the list should lead to a pharmacist or clinician review, not an unsupervised decision to stop a cardiovascular medicine or change the anticancer prescription.
Preserve laboratory context and recent complications

A useful handover allows the next clinician to continue the work with a clear account of what happened, what remains uncertain, and who will address it.
