Patients can contribute by describing the actual change: diarrhea frequency and characteristics, reduced intake, weight loss, rash distribution, or a new limitation in walking or breathing. Significant deterioration should be communicated directly. Severe diarrhea, bleeding, breathing difficulty, or possible infection warrants timely local assessment rather than waiting for an overseas hospital to finish reviewing documents.
Corticosteroids remain central to systemic treatment of acute GVHD
Different degrees and patterns of acute GVHD do not all require identical treatment. Where systemic treatment is indicated, corticosteroids remain the recommended first-line foundation. The 2026 ASTCT review does not endorse routinely adding another systemic agent to corticosteroids for all newly diagnosed patients merely because newer drugs are available.[4]

Inadequate response, deterioration during a taper, and inability to tolerate treatment describe different problems. Dry eyes may limit reading, mouth pain can reduce food intake, skin or fascial changes can restrict movement, and pulmonary involvement needs its own evaluation. Goals can include controlling active disease, relieving symptoms, preserving function, and reducing treatment burden. These outcomes may improve at different speeds.[5]
Local treatment can be important for limited manifestations, while more extensive or severe disease often requires systemic therapy with organ-specific care. Eye treatment, oral care, or rehabilitation is not an interchangeable substitute for systemic treatment, but each may address a problem that the systemic prescription alone does not resolve.
At a review, describe symptoms and functional limitations separately. Improvement in one result does not establish complete recovery of every organ. Conversely, a remaining tight area of skin or persistent dryness does not automatically prove that all treatment has failed. The distinction between active inflammation and established damage helps the team set realistic goals and decide what to change.

Nor can response percentages from unrelated studies reliably rank all available medicines for one person.
Identify the exact cellular product and patient population
In 2025, China NMPA announced conditional approval of amimestrocel injection for steroid-refractory acute GVHD with predominant gastrointestinal involvement in patients aged 14 years and older. This is a particular umbilical cord-derived mesenchymal cell product, not an authorization for any stem cell preparation to treat every form of GVHD.[8]
FDA's remestemcel-L-rknd approval concerns a different, bone marrow-derived mesenchymal stromal cell product for steroid-refractory acute GVHD in children aged two months and older.[9] Product name, source, age, disease type, and previous treatment requirements need to remain distinct when reading international reports.
The United States TREGZI information issued in 2026 describes a matched-donor transplant pathway with myeloablative preparation in adults with hematological malignancies, addressing hematopoietic and immune reconstitution and improved chronic-GVHD-free survival.[10] It should not be presented as a general rescue infusion for anyone who already has GVHD.
A newer option still needs an assessment and monitoring plan
For previously treated chronic GVHD, FDA has approved axatilimab-csfr in a population defined by prior systemic treatment and weight criteria.[11] That regulatory decision does not verify Chinese access or a treatment slot. A consultation should identify the actual option under consideration and explain how its evidence relates to the patient's condition.
For any chosen treatment, agree how benefit and adverse effects will be assessed. Current United States ruxolitinib labeling, for example, addresses blood-cell reductions, infection, and interactions. Different release formulations are also reflected in the 2026 label.[12] Patients should not convert tablet numbers or substitute formulations independently; the prescriber and pharmacist must review the exact product.
A clinical study may create another possible pathway, but screening depends on the protocol, organ status, and prior treatment. A publication or registry entry is not confirmation of an available place. Necessary current care should not be interrupted by the patient while waiting for a research possibility to become clearer.
Infection prevention and nutrition belong in the treatment plan
Suppressing GVHD and managing infection risk must proceed together. Keep records of preventive medicines, recent infection testing, organisms already treated, and monitoring still underway. Fever or a new breathing problem should not automatically be attributed to GVHD, and a previous negative test does not settle every later episode.[3,4]
With gastrointestinal disease, inadequate intake and weight loss can make recovery more difficult. Nutrition support should reflect bowel tolerance and the current clinical situation. A patient should not adopt prolonged fasting or another person's restrictive diet as a self-prescribed GVHD treatment. Pain, dryness, or swallowing problems that limit eating need to be reported explicitly.
Chronic manifestations may require rehabilitation and skin, oral, eye, or other specialist input. MSK's dedicated clinic provides an example of multidisciplinary and nutritional assessment as part of GVHD care.[13] This helps illustrate the range of services a patient might need; it does not establish that every Chinese center has the same configuration. Ask about the specialties relevant to the organs actually affected.
Define the purpose of a consultation in China
A useful referral might seek confirmation of classification and organ involvement, advice after an inadequate response, assessment of a named treatment, or a coordinated plan for longer-term organ care. For an unstable patient, immediate local care may be the priority while the hospitals discuss whether transfer is feasible. A remote appointment cannot replace treatment of a rapidly worsening complication. Travel planning should follow the clinical situation and acceptance arrangements, with continuing medication and support addressed explicitly.
The record package should include the transplant type, donor information and date, prophylaxis, organ assessments, systemic and local treatment history, and infection investigations. Preserve actual corticosteroid instructions and taper dates. A statement that the patient has been on steroids for a long time is not enough to establish response, exposure, or the burden of treatment.
Without an individual plan and a formal hospital estimate, a complete RMB price cannot be established. Ask for costs covering investigations, medicines or a specific cellular product, admission, and organ support, then account for continuing care at home. Because GVHD treatment often requires repeated reassessment, the value of the referral depends on identifying who will deliver care and interpret results after the initial opinion.
